Affix Signed Passport Size Photograph

When a form says “affix signed passport size photograph”, it wants your own signature on the photo. Here is where it goes, on paper and online, with a tool to sign the photo for upload.

On a paper form

  1. Use a recent photoPassport size, 3.5 × 4.5 cm, light background, face clearly visible.
  2. Paste it inside the boxUse glue, not staples or tape. Keep the photo straight.
  3. Sign across the lower edgeStart on the photo below the chin and finish on the paper, so the signature crosses the edge.
  4. Use your usual signatureThe same one you put at the end of the form, in blue or black ball pen.

On an online form

There is no paper to sign across, so the signature goes on the lower part of the photo itself. Upload your photo above, tap the button to add your signature and the date, and download it at the size the form lists. The tool places the signature on the shirt area so it never covers your face.

If the form says “self attested” rather than “signed”, it wants the words “Self Attested” as well. Use the self attested photo maker for that.

Wording you may see on forms

Form saysWhat to do
Affix signed passport size photographPaste the photo, sign across its lower edge
Affix recent passport size photographPaste the photo; signing is optional unless stated
Photograph signed acrossSignature must cross from the photo onto the paper
Do not sign on the photographPaste only; sign in the box below

Exam forms often word it differently; see photo duly signed by the candidate for that case.

Questions

What does “affix” mean on a form?

To stick or paste. “Affix signed passport size photograph” means paste a passport size photo in the box and sign it. On an online form it means upload a photo that already carries your signature.

Should the signature be on the photo or on the form?

On a paper form, sign so the signature runs across the lower edge of the photo onto the paper. Half the signature is on the photo and half on the form, which shows the photo was not swapped later. Never sign over your face.

Do I also need to write “Self Attested”?

Only if the form says so. “Signed photograph” asks for your signature, not the words. If the instructions say “self attested photograph”, add the words as well.

Can I use an old photo?

Most forms ask for a recent photo, usually taken within the last six months, so that it matches your appearance at verification.

Should I glue or staple the photo?

Glue it. Staple pins damage the photo and the form, and many offices reject stapled photos.
